Insurance 
As a courtesy to you, Lisa C. Evans Heath, LPCS, will file your in-network insurance claim.
 
Rates are determined by your insurance company plan benefits, type of services provided, and length of session.
 
Please contact your insurance company before the initial visit, as they are responsible for explaining your mental health benefits, limitations of coverage, and restrictions. Your insurance company will also explain any deductibles, co-insurance, or co-payments which you are required to pay at the time of service. To contact your insurance company, call the number located on the back of your insurance card under Customer Service, Mental Health benefits (MH), or Behavioral Health.  

When calling your insurance carrier or plan administrator request a call reference number and the name of the person to whom you are speaking.  Insurance companies and plan administrators are responsible for the accuracy of the information they provide. We do not accept responsibility for explaining your benefits nor do we guarantee insurance claims will be paid.  
 
Payment
 
Payment is due at the time of service.
 
For your convenience, you may use Zelle through your banking institution to pay your balance to Lisa C. Evans Heath LPC-S use 972-522-8448 for the number to send payment. We will never have access to your credit card, debit, or bank account information. You may also send a check to Lisa Evans, LPC-S, 211 Masters Ave., Suite #100, Wylie, TX  75098.  If you wish to make a credit card payment you can do so through your cash pay option to Lisa C. Evans, MA. Inc.   There is also a 15% balance charged per month for an outstanding balance until all payment is made.
